In this remote and adult day center based role, you'll play a critical part in a team dedicated to enhancing the lives of seniors through comprehensive and compassionate healthcare. You'll use your expertise in Internal Medicine, Family Medicine, Hospice, or Urgent/Emergency Care to lead advanced care planning and medication optimization.

What You'll Do:
- Provide comprehensive medical examinations and treatments to patients
- Drive healthcare initiatives focused on advanced care planning and medication optimization to enhance patient outcomes.
- Collaborate with healthcare professionals, including Nurse Practitioners and specialists, to deliver integrated care.
- Mentor and guide clinical staff in managing complex patient cases.
- Oversee clinical standards and safety measures to ensure effective and compliant patient care.
- Participate in quality improvement activities to refine treatment practices and outcomes.
- Support and mentor of other providers.
What You'll Bring:
- Valid Pennsylvania medical license.
- Board certification in Internal Medicine, Family Medicine, or a related field.
- Experience in Hospice, Urgent, or Emergency Care preferred.
- Familiarity with VA healthcare system and strong geriatric care background highly desirable.
- Proven ability to supervise Nurse Practitioners and manage health programs.
- Excellent diagnostic and leadership skills.
