The Home Health Nurse Practitioner provides in-home and virtual care for patients with chronic kidney disease (CKD) or end-stage renal disease (ESRD) in East and Southwest Virginia. This hybrid/remote role operates Monday to Friday, 8 am to 5 pm, offering a work-life balance with no weekends or on-call duties. The position requires an active WV state license, BC and DEA certifications, and at least two years of direct patient care experience.
