WCS Healthcare is currently seeking an Case Management Nurse for a contract assignment at a Prestigious Hospital in Northampton, MA.
Job Function:
- Day Shift.
- 8am to 4:30pm.
- Assess new patients.
- Update and revise patient health care plans as needed.
- Educate patients and families about health care plans and options.
- Make clinical decisions for routine patient care.
- Accurately document and submit medical records.
- Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.
Job Requirements:
- Graduate from an accredited School of Nursing or Bachelor's Degree in Nursing (BSN).
- Current RN license for the state that you are working in (or willingness to obtain).
- BLS certification from the American Heart Association.
- CCM certification preferred.
- 2 years of acute care experience in this specialty.
