Location: Humboldt County, CA; northernmost part of the state Start Date: ASAP End Date: Ongoing; must make a minimum 3 month commitment Schedule/Shift: Full-time, Monday - Friday, 8am - 5pm; 3 month minimum commitment; Extensions are possible and providers willing to stay long term will be given preference Volume: 15-20 patients per day Requirements:

• Active, unrestricted License from Any U.S. State or Territory
• Board Certified or Truly Board Eligible Family Practice or IM Doctor
• Clean Background, DEA, Malpractice, and NPDB
• Able to commit to 3 months, full-time
• Must submit updated CV with response to posting
• Must be committed to rural, tribal and underserved populations healthcare
