We are seeking a Nurse Practitioner to conduct Evaluation & Management (E&M) visits for Medicare patients in their homes. This role involves direct patient care, travel between patient residences, and timely documentation of visits.

Key Requirements:

• Active, unrestricted NP license (FNP or Adult NP preferred)
• Board certification as an NP in relevant specialty
• Experience with E&M visits and/or home health strongly preferred
• Reliable transportation and valid driver s license
• Strong clinical assessment and documentation skills
• Ability to work independently and manage daily schedules
• Comfortable using EMR systems for visit documentation
