• Specialty: Family Medicine or Internal Medicine

• Setting: Rural Health Clinic + Hospital + Long-term Care

• Schedule: Full-time, permanent

• Team: 2 Physicians + 6 Advanced Practice Providers

• Provide comprehensive, full-scope family care across outpatient, inpatient, and long-term care settings

Candidate Requirements:

• Board Certified/Eligible in Family Medicine or Internal Medicine (within 3 years of residency)

• Active IL license (or ability to obtain)

• Clean malpractice history preferred
