Coverage Needed:

• As soon as credentialed - ongoing
• Schedule: Preferably a variation of day shifts, OB shifts and OR call
  • OR call 4p-7a possibly once a week
  • OB call 7a-7a (negotiable)
  • would consider a 5 days a week dayshift

Clinical Information:

• Typically 3-5 cases per day
• If taking call would need to do epidurals/spinals for OB
• Art line/central line placement
• EMR: EPIC

Required:

• CV must be in Month / Year format
• Must have license in hand at time of submission, or proof that the license application has been submitted
• Any gaps greater than 30 days will require an explanation on the CV
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
• Active IN License and DEA (or one easily transferrable)
• EPIC experience - Preferred
• Board Certified: Anesthesia
• CV Anesthesiologists need to be TEE certified
