Physical Therapist (PT) – PACE Program | Albuquerque, NM
MedAdventures is seeking an experienced Physical Therapist to join a PACE (Program of All-Inclusive Care for the Elderly) center in Albuquerque, NM. In this role, you’ll support an interdisciplinary team to help participants maintain mobility, independence, and overall quality of life.

What You’ll Do:
- Provide individualized physical therapy services to PACE participants
- Develop and implement treatment plans focused on functional mobility, safety, and independence
- Collaborate closely with nursing, therapy, and interdisciplinary team members
- Educate participants and caregivers on exercises and mobility strategies
- Maintain accurate and timely clinical documentation
Requirements:
- Active New Mexico PT license
- Experience with geriatrics, home health, outpatient, or PACE preferred
- Strong communication, critical-thinking, and teamwork skills
- Passion for delivering compassionate, participant-centered care
