Candidate Requirements
- Licensed APRN in Florida and graduate of an accredited nursing program
- 1-3 years of primary senior care experience preferred
- Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension
- Proficient in EMR documentation with strong organizational skills
- A natural communicator with genuine compassion and strong follow-through
- Passionate about senior-focused, value-based care and patient advocacy
