This role will primarily support the Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program, delivering field-based care to adults (ages 18-59) facing serious behavioral health and substance use challenges.Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program provides outreach-based treatment services to individuals with serious behavioral health challenges including substance use disorder (SUD).
