Candidate Requirements
- CV Details: Candidate CVs need to include month & date on their work experience with all gaps in employment accounted for.
- Chart Closure: Within 72 hours
- Schedule: 18 patients per day (PPD), with a minimum of 18 PPD
- Work Hours: Monday-Friday, 8 am to 5 pm
- Patient Mix: Diverse mix of pediatrics and adults
- Decision Timeframe: A decision is typically made within 7-14 days
- Acceptance of DO: We will also accept Doctors of Osteopathy (DO)
- Experience: Advanced Practice Clinicians (APCs) with 3 years of experience in a similar setting are preferred (see other AR)
- Board Eligibility: Board Eligible (BE) candidates will be considered if truly BE (5 years or less since residency)
- DEA Certification: Active DEA needed before the start date
- Credentialing Time: Credentialing takes 30-90 days
Immunization and Health Requirements
- Required Immunizations: The following immunizations are required for credentialing (before the start date):
- Flu Vaccine
- TB Status/X-Ray Results (results must be within the 90 days before the start date)
- Positive MMR Titer or Complete Immunization Series
- Positive Hep B Titer or Complete Immunization Series
- Positive Varicella Zoster Titer or Complete Immunization Series
- Tdap Immunization (must be within the last 10 years)
- Covid-19 Vaccine is no longer required
Agreement Details
- Notice Period: Vendor agree to a 30-day notice for ending all assignments.
