Case Manager, Social Services

Opportunity to join a dedicated healthcare team providing patient- and family-centered care at Memorial Healthcare System. Case Manager, Social Services; full-time schedule likely; location: Florida (Memorial Healthcare System); role involves care coordination, leadership, and complex case management; contract: permanent (not specified). - Licensed Clinical Social Worker (LCSW) in Florida preferred/required for Dialysis, Transplant, Cystic Fibrosis and ED mental status exams - Minimum 1 year experience - Degree in Psychology, Mental Health, Marriage & Family or related social services field - Strong critical thinking, communication and leadership skills

Complexity of Work: This role requires critical thinking skills, effective communication, decisive judgment, and the ability to build and foster positive relationships. The incumbent must be able to lead others and take appropriate action when required.

Required Work Experience: One (1) year of experience required.

Other Information: Licensed Clinical Social Worker in Florida is preferred. JDCH Specific: Licensed Clinical Social Worker license is required for the following areas: Dialysis, Transplant, Cystic Fibrosis and mental status exams in the Emergency Department.Additional Education Info: Psychology, Mental Health, Marriage and Family, and/or related social services field.
