We are looking for a Board Certified Primary Care physician to join our established PACE Program. If you have a CAQ in Geriatric Medicine and at least 1 year of experience caring for the Geriatric population, we want to hear from you!

Responsibilities:

• Provide primary medical care to participants
• Assess participants and develop a plan of care in collaboration with the interdisciplinary team
• Provide direct participant medical care and evaluate the effectiveness of the plan of care
• Function in a collegial relationship with other health care professionals
• Make independent decisions regarding medical needs and health regimens

Qualifications:

• Board Certified in Primary Care
• CAQ in Geriatric Medicine
• Minimum of 1 year of experience caring for the Geriatric population
