JOB REQUIREMENTS
• Licenses: Illinois license (can apply if not currently held)
• Board: Board Certified or Board Eligible in Pediatric Otolaryngology
• DEA: Active DEA Certification required
• Certifications: ACLS, BLS, PALS
• Experience: Minimum 1 year in specialty
• Other Qualifications: Proficiency with Epic EMR

• Participate in coverage for the CHOI Pediatric ENT Clinic
• Support dedicated ENT team by filling in during service gaps or provider PTO
• Take night call coverage as needed
