• M-F 8 am - 5 pm with some on-call required
• 15 - 16 patients per day
• Geriatric population focus
• Chronic care management including COPD, kidney disease, diabetes
• Complex patient panel requiring detailed charting
• Hospital privileges required
• BLS certification required
