Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree required.
- Current State License as a Physical Therapist in MA or other applicable state required prior to start.
- Proof of current CPR certification required.
- Current valid driver’s license, reliable transportation and proof of insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home care experience preferred.
- Ability to perform patient assessments, develop and revise Plans of Care, document progress notes and collaborate with interdisciplinary teams as part of standard clinical practice.
- Ability to lift, bend, pull, push, stand for prolonged periods and travel locally as required by home health visits.
- Must read, write and speak fluent English.

Job Summary
A registered physical therapist who provides therapy to patients across age groups in the home health setting. Plans, implements and evaluates patient care to restore or maintain patient well being. Conducts clinical assessments, documents Plans of Care and progress notes, and revises treatment plans based on evaluation and ongoing assessment. Collaborates with physicians and interdisciplinary team members. Provides patient and caregiver education on transfer techniques, use of assistive devices, and home-based adaptive equipment. Applies infection control and universal precautions. Accepts assignments consistent with education and competence. Performs other related duties as assigned.
