About You: - Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) degree from an accredited dental school; Or currently attending an accredited dental school to obtain a degree for DDS or DMD.
- Current, valid dental license to practice in the state or ability to obtain.
- DEA certification
- CPR/BLS certification (or ability to obtain)
