The facility operates with a telehealth model and manages approximately 350 patients through a team-based approach to hospice and palliative care.
Opportunity:
Physician - Hospice and Palliative Medicine
Job Details:
• Full-time telehealth position in Portland, serving both Oregon and southwest Washington patients• Monday-Friday schedule from 8am-5pm with flexible lunch breaks• On-call responsibilities include 4-6 nights per month plus approximately 1 weekend per month• Shared call between Portland and Medford locations• Provider will manage a portion of approximately 350 patients• Responsibilities include interdisciplinary team meetings, consultations, medication management, telehealth visits, and critical interventions• Weeknight calls vary from minimal activity to multiple admissions and complex patient management• Weekend coverage may include admissions and managing complicated patients, sometimes with NP support• MUST have active OR license, OR DEA, WA license, and WA DEA• Board Certification required (Board Eligible accepted if within 5 years of residency)• Must be BC/BE in Family Medicine or Internal Medicine with ample hospice experience (Hospice certification preferred)• BLS certification required• No estimated credentialing timeframe indicated

Competitive hourly rates with weekly pay and malpractice included.
