- Schedule: 8:30am - 6:30pm (30-minute unpaid lunch), four 10-hour shifts per week with a rotating day off (Tuesday, Wednesday, or Thursday)
Responsibilities and Duties
- Provide outpatient care for acute and chronic conditions
- Conduct wellness exams and preventive screenings
- Manage chronic diseases such as diabetes, hypertension, heart disease, COPD, and asthma
- Address minor injuries and wound care
- Deliver pediatric care and immunizations
- Provide geriatric care and manage chronic conditions
- Support behavioral health needs including depression and anxiety
- Manage substance use disorders through the MaT program
- Coordinate referrals and specialty care, including telehealth
- Deliver culturally sensitive care for the Native community
- Educate patients on lifestyle, prevention, and self-management
- Document care accurately in the electronic health record
- Support after-hours urgent care needs
- Collaborate with lab, pharmacy, and other departments for patient follow-up
- Participate in quality improvement initiatives and PCMH standards
Additional Information
- Active Nurse Practitioner license in any state required
- Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications required
- Experience working with pediatric, adult, and geriatric patient populations preferred
