Seeking an experienced Pulmonology Nurse Practitioner (APRN) to provide diagnosis, treatment, and interdisciplinary care coordination for patients in skilled nursing and long-term rehabilitation facilities, focusing on pulmonary and acute care across multiple specialties. The role aims to reduce hospital readmissions, improve patient outcomes, ensure quality documentation, and support facility efficiency within a multi-state post-acute care practice. Position is based with NextGen Post Acute Care and includes openings in Brady, TX, supporting care in skilled nursing facilities.
NextGen Post Acute Care is a multi-specialty practice group specializing in pulmonary, infectious disease, cardiology, and post-acute care. We currently have over 100 providers delivering specialized care to patients in skilled nursing facilities across Florida, Ohio, Tennessee, and now in Pennsylvania and Texas.
A core aspect of our program is reducing hospital readmissions, ensuring responsible stewardship, and optimizing patient care within these facilities. Our Corporate Headquarters is in Hallandale Beach Florida.

We are seeking a compassionate and experienced APRN to join our growing healthcare team, providing high-quality care in skilled nursing and long-term rehabilitation facilities. This role involves diagnosis, treatment, patient management, and interdisciplinary care coordination across a range of specialties, such as pulmonary and acute care, infectious disease, cardiovascular, pain management, psychiatry, and physical rehabilitation.
