SSM Health Saint Mary's Hospital in Richmond Heights, MO is seeking an experienced Advanced Practitioner (NP or PA) to join our Transitional Care Clinic (TCC) and Infectious Disease Department. TCC provides continuity care with a focus on health and wellness for all patients, regardless of insurance or demographics. These are one-time, post-discharge visits.

- Schedule:
- Full-Time
- Monday-Friday, 8a-5p.
- NO nights/weekends/holidays or call!

Your Qualifications:
- Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
- Ability to obtain PA/NP licensure in Missouri
- Internal medicine and emergency medicine experience is highly preferred
- Excellent interpersonal and communication skills
- Ability to work independently and collaboratively within a multidisciplinary healthcare team
