Job Summary:
We are seeking a dedicated Occupational Therapist to provide high-quality home health rehabilitation services. In this role, you will assess, evaluate, and deliver medically prescribed occupational therapy programs to help patients restore and maximize functional abilities, improve daily living skills, and prevent further physical limitations. You will support patients in achieving their highest level of independence while aligning with organizational goals and best clinical practices.
Territory Coverage:
San Jose, Sunnyvale, Mountain View, Los Altos, Palo Alto, Stanford, Menlo Park, East Palo Alto, Atherton, Redwood City, and nearby surrounding cities as needed.
Responsibilities:
- Evaluate, develop, and implement individualized occupational therapy treatment plans.
- Improve patients’ functional abilities and activities of daily living through therapeutic interventions.
- Monitor progress, adjust treatment plans, and document care within the EMR system (Homecare Homebase HCHB).
- Collaborate with the interdisciplinary care team to ensure coordinated patient support.
- Provide holiday coverage as needed.
- Perform additional duties consistent with training and patient care needs.
Job Details:
- Shift: Days, Monday–Friday, flexible hours (8:30 AM–5:00 PM)
- Contract Length: 13 weeks
- Experience Required: Minimum 1 year
- License: Active state OT license required
