This locum tenens position seeks an Obstetrics & Gynecology Physician to provide urgent and emergent care, including deliveries and GYN emergencies, primarily at Longmont United Hospital. The role requires 24-hour shifts, EMR proficiency (EPIC), and adherence to credentialing and travel policies. - Description: Summary: Longmont United Hospital is looking for a locum OB/GYN to help with Urgent November holiday coverage for emergent GYN and OB deliveries.BID DUE DATE: Nov 24th at 5p EST! Rate caps for submission:Malpractice Must be included in rates.Orientation 200 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours On Call Daily 3300 - Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate - 4950 - Per 24 hours to include 4 hours gratisCall Back 400 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms. Holiday Call Back 600 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms. Average worked Callback Hours: 6.62 hoursSchedule/Availability Requirements:Nov 27 7a - 29 7aongoing dates will be determined as schedules are released.Details:24 hour call 7a-7a Total of 9-10 shifts needed per month for all locums. Site will not schedule providers more than 2-3 day stints. Setting: Inpatient Emergency Room Call Trauma Level IIIFTE: 2.0Required Procedures/Job Duties: GYN Emergencies Obstetrics including C-section.EMR: EPICSupport Staff: RNReason for Coverage: Gaps in coverageDealbreakers Do Not Present: Must be willing and able to work 24 hour shifts.EMR Required Training:Credentialing Preapp and full app must be completed and submitted to move provider into applicant in training to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account. Module learn lessons Must be completed prior to virtual EMR training to attend the training.EMR Virtual PPL training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day. First Day Details - Site requires completion of EMR Modules and training before first day details can be released. All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.What are the # of years of epic experience they have?When was the last time MM/YYYY that they used Epic?What is the name of the last facility the provider used EPIC?Can you provide me with a photo large enough to add to a badge and not become pixelated? - Please attach photo in documents sectionCan you disclose the provider's time zone for scheduling the virtual epic training?We need the provider's best phone # to use for call shifts while working. Can you disclose?We need the provider's best email to use for programs while working onsite and the preapplication. Can you please send us their personal email address? Travel Preferences:Preferred Lodging: Providers must be willing to stay at the Hampton Inn - 850 S. Main st. Longmont Co - . When booking they must reference Centura Health or LUH to receive the prenegotiated rate between the facility and hotel. Providers are required to book at the preferred hotel when room are available. If rooms are not available they must follow the CommonSpirit Health travel policy. Air Travel: Local/drive-in candidates only - Highly PreferredPlease detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.Credentialing Information:Timeframe: 60 days temps / 120 days fullCredentialing with other facilities? YesIf so where: LUH SAN - Not required for coverage just would be good to be privileged at both.Frequency of work at each facility: Most if not all with be at LUH.Requirements: Self inquiry NPDB report must be provided and CO license or Active IMLC to release preapplication.Requirements: Preapp and full app must be submitted within 72 hours of receipt of initial app to allow enough time for AD account creation for EMR requirements. Once both are submitted provider is moved to applicant in training which initiates AD Account creation for provider EMR account and can take up to 5 days before release of EMR training requirements.
