Opportunity:
PT - Physical Therapist
Job Details:
• Work Monday through Friday from 8:00 AM to 5:30 PM
• Guaranteed 37 hours per week with flexibility to work up to 40 hours
• 13-week contract assignment starting as soon as possible
• Rotate one Saturday every 6-8 weeks with compensatory day off during the week
• Occasional travel to other branches as needed with mileage reimbursement at government approved rate
• Can be placed on call for low census situations with minimum 2-hour call back pay
• Contract eligible for 36 hours of unbillable low census hours
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient population with orthopedic, neurological, and functional mobility diagnoses
• Document patient care to meet regulatory and reimbursement requirements
• Meet productivity expectations averaging 4.5 visits per day
• Use laptop for point of service documentation, NetSmart EMR system, and cell phone for GPS and communication
• Required certifications: AHA BLS, Wisconsin state licensure, NPI number
• Required education: Bachelor's degree minimum, Master's degree in Physical Therapy preferred
• Home health experience required with knowledge of Physical Therapy standards and practices
• Must be hardworking, resilient, manage change well, and have strong social skills
