Practice Overview:

• Dedicated hospice team serving patients for 40 years.
• Inpatient Hospice Unit at main campus- 7 bed unit
• Free standing 16 bed hospice currently under construction just minutes from main campus
• Provide support to nursing and interdisciplinary staff by providing oversight for home hospice and in-home palliative care
• Monday through Friday schedule
• Service Line supported by dedicated:
  • Physicians and Advanced Practitioners
  • Licensed Clinical Social Workers
  • Chaplains
  • Designated nurse staffing for after hour, weekend & holiday care

Working together with Nurse Practitioners like you:

• Board Certified FNP or AGACNP
• Active Georgia license or ability to obtain unrestricted license
• Passion for quality and patient care
• Experience in Hospice or Palliative Care preferred
• Functions well in a team environment, contributing to and supporting organizational goals and initiatives
