What You ll Do:
- Deliver top-notch care to post-acute patients Monday through Friday (weekends are for you!).
- Manage patient assessments, care plans, and follow-ups.
- Collaborate with a supportive interdisciplinary team that believes in quality care (and quality coffee).
- Document care with precision because every patient deserves a note-worthy experience.
