Registered Nurse - Hospice - Per Visit

Position Summary
The Per Visit Nurse is responsible to provide a patient centered approach, skilled nursing or other therapeutic services in the home or other appropriate place. The Per Visit nurse promotes health and prevents both mental and physical illness, helping patients obtain other necessary community services.
Minimum Qualifications:Graduate from a hospital school of nursing with National League of Nursing accreditation at time of graduation or Graduation from a university public health nursing program approved by the National league of Nursing. Registered Professional Nurse with a current license in the State of Connecticut. One year public health nursing experience and/or general nursing experience.
