We are seeking a compassionate and highly organized Case Management/Clinic Care Guide (RN) to join a growing, patient-centered care team. This role is central to delivering a high-quality healthcare experience by guiding members through specialist referrals, diagnostic testing, and ongoing care coordination. The Clinical Care Guide supports patients both in-person and virtually, serving as a trusted advocate throughout their healthcare journey. This position emphasizes patient experience, privacy, compliance, and collaboration in a dynamic healthcare setting.
What You’ll Do:
Support members in-person and virtually with real-time consultation, education, and care coordination
Schedule and manage specialist and diagnostic referrals, ensuring follow-through and follow-up
Clarify and explain referral recommendations and diagnostic testing needs
Own the end-to-end referral and specialist navigation process as needed
Play a key role in improving health outcomes and contributing to cost savings
Serve as a visible, empathetic partner to patients and families
Support members with chronic conditions through education, reminders, and ongoing engagement
Lead and support quality initiatives, including patient outreach for gaps in care and clinical needs
Manage and integrate office workflows and processes
Handle service and scheduling requests on behalf of members
Inform and equip the care team with referral requirements and updates
Build relationships with local healthcare providers, community organizations, and partners
Perform other duties as assigned
Schedule & Work Environment
100% on-site at the care center location
May include extended hours
May involve movement between clinic locations as needed
What You Bring
Case Management experience
Clinic, Primary Care, and/or Urgent Care experience
Active Registered Nurse (RN) license in the state of Arizona
BLS certification
Active AZ Fingerprint Clearance Card (not required initially)
Experience with Electronic Medical Records, including Medicare documentation
Strong customer service and patient-centered care experience
Excellent communication skills across multiple modalities
Highly collaborative, empathetic, and patient-focused
Exceptional organizational and time-management skills
Strong attention to detail and commitment to confidentiality
Ability to work independently while thriving in a team-based environment
Proficiency with additional healthcare software platforms
High degree of independence, flexibility, initiative, and commitment
