Job Details
Seeking an experienced Travel RN Case Manager with home health case management experience. The RN Case Manager will oversee patient care across the continuum, coordinate interdisciplinary services, manage visit utilization, and ensure high-quality, cost-effective care for a geriatric patient population.
Requirements
- Active California RN license
- BLS
- Minimum 1 year RN experience
- Home Health Case Manager experience required
- Experience with care coordination, discharge planning, and utilization review
- Epic charting experience preferred
