- Board Certification
- State License: Highly preferred, open to IMLC w/ LOQ in hand.
- CV surgery Fellowship-trained.
- Both BLS & ACLS (Should be in place, but the training can be done on-site if necessary.)
- Malpractice status: (Required to disclose to obtain for temporary Privileges.)

- Reason for Coverage: separation with current employed Surgeon
- Deal Breakers: Malpractice history, board and or license practice restrictions (Should have board and license to practice, malpractice Is case by case.)
- Are both Thoracic and Cardiac required? yes
- How many hearts/ year is the program doing? 100 possible
- Is there a minimum number of heart surgeries that must be performed from a candidate proficiency perspective? Will review case by case but should have enough to maintain licensure and board.
- Is the surgeon required to harvest veins or will a PA be present? Should be able to harvest own veins, hospitals may provide PA depending on volume.
