As a Physical Therapist in the PACE program, you’ll play a vital role in assessing, planning, and delivering individualized care to participants. From therapeutic exercises to group programs, you’ll help improve mobility, enhance quality of life, and support participants in maintaining their independence.
What the Physical Therapist Will Do:
- Evaluate participants’ physical condition and functional abilities to create personalized care plans.
- Develop and implement individual and group exercise programs.
- Provide participant and family education on rehabilitation needs and techniques.
- Collaborate with an interdisciplinary team to coordinate and optimize care delivery.
- Supervise program aides and ensure the quality of care meets established standards.
- Conduct home visits and training as necessary to support participants’ needs.
What We’re Looking For:
- Education: Bachelor’s degree in Physical Therapy (pre-2000) or Master’s degree (post-2000).
- Licensure: Current Physical Therapist license for the required state.
- Experience: 1+ years of experience with frail or elderly populations preferred (training provided if needed).
- Skills: Strong clinical knowledge of geriatric care, effective communication, and teamwork.
- Reliable transportation, valid driver’s license, and required liability auto insurance.
- Up-to-date medical clearance and immunizations.
Work Environment:
You’ll work in a dynamic Adult Day Health Center, collaborating with a wide range of clinical and social service professionals. The setting includes a rehabilitation gym and primary care clinic with a supportive, team-oriented atmosphere.
