Job Summary
Cooperative Health is seeking mission-minded, compassionate Family Nurse Practitioner s (FNP) to join its dedicated medical staff. The ideal candidate will provide comprehensive pediatric and adult or adult oriented primary, preventative, and episodic care. The Nurse Practitioner is a part of a dynamic care team that works in a holistic manner addressing each patient s physical, emotional, social, and mental health needs to promote well-being and deliver extraordinary outcomes.
Positions are available on both full-time and part-time bases across well-established rural and urban practices. All FNPs work in an office-based, Patient-Centered Medical Home (PCMH) setting. Integrated support from in-office Behavioral Health and Social Service team members is available at many locations.
