Overview
An excellent opportunity for a Medical Director specializing in Geriatric Medicine in West Springfield, MA. The role follows the Program of All-Inclusive Care for the Elderly (PACE) model, designed to help seniors with chronic needs maintain independence and receive comprehensive medical and supportive care within their community.
Practice Highlights
• Provide medical direction and supervision of participant care and oversee clinical outcomes.
• Lead and guide Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
• Oversee the Quality Assurance Performance Improvement (QAPI) program.
• Develop and maintain medical policies, procedures, and standards.
• Perform histories, physicals, reassessments, and manage treatment plans.
• Participate in interdisciplinary team meetings and family conferences.
• Educate participants and caregivers on chronic conditions and health maintenance.
• Perform physical exams, order/evaluate diagnostics, and document results.
• Participate in the on-call rotation as required.
Candidate Requirements
• Active Massachusetts medical licenseRequired
• Board Certification in Internal Medicine or Family PracticeRequired
• Board Certification or experience in GeriatricsPreferred
• CDS Certification and DEA RegistrationRequired
• Previous supervisory experienceRequired
• Minimum 1 year of experience with a frail or elderly populationRequired
• Strong skills in physical assessment and chronic disease managementRequired
• Desire to work in an innovative elder care settingRequired
