Job Details:

• Specialties: FNP, ANP, GNP, PA
• Setting: In-home health & wellness assessments (travel required within state)
• Schedule: Mon Sun 7 am 7 pm 5 8 visits/day (first exam can be scheduled as early as 8am and last exam for 6pm.)
• Ideal Availability: At least 2 full days per week (must include 1 weekday; weekend-only not accepted)
• Patients: Primarily geriatrics
• Duties: H&P evaluations, vitals, med review, finger stick, urine sample, fecal kit drop-off
• Documentation: Proprietary EMR system (iPad provided)
• Employment Type: Credentialing days (can be as quick as 2-3 weeks, modules will take approximately 10-12 hours to complete depending on your reading speed.)

Compensation & Incentives:

• $88 per completed in-home assessment (varies by state and by number of exams per month)
• Weekly bonus: $120 if you are available for 30+ visits/week and complete 15+ visits/week
• Lab add-ons pay: $5+ per lab completed (varies by type)
• Note: Cancellations (1 2/day possible) are unpaid, so scheduling 6 8 visits/day is recommended

Travel & Lodging Benefits:

• Rental Car: Provided for providers available for 30+ visits/week (local & non-local)
• Flights: Covered for providers working M F for at least 2 consecutive weeks
• Hotel: Provided if assignment is 75+ miles from home address
• Urban Assignments: In metro areas, rideshare reimbursement (Uber/Lyft) may be provided instead of rental car

Requirements:

• Licensure & Credentials: Active NP or PA state license, NPI #, CAQH (can apply during credentialing). Must not have opted out of Medicare/Medicaid.
• Background Info (for presentation): Middle name, DOB, home address, NPI, and languages spoken. Must include completed Phone Screen when sending name clear.
• Insurance: Personal auto insurance with $100K coverage and acceptable driving record.
• Training: 10-12 hours of online modules (unpaid) must be completed prior to scheduling. Includes EMR/iPad training and safety protocols.
• Equipment (provider-supplied): White lab coat, stethoscope, ophthalmoscope, monofilament (10g), 128Hz tuning fork, reflex hammer, BP cuff, pen (blue/black), portable scale (up to 400 lbs), tape measure, and pulse oximeter.
• Wardrobe: Scrubs or business casual. PPE provided.
• Travel Radius: Willing to travel at least 75 miles from home. Candidates should provide current address and preferred counties.
• Scheduling Commitment: Must provide monthly availability 4 weeks in advance. Any changes require 2 weeks notice. Failure to honor scheduled patient visits may result in removal from the assignment ( one strike and out policy).
