This role is for a full-time Geriatrics Physician responsible for providing specialized care to complex older adult patients across multiple nursing facilities and advanced care programs. The position involves supporting an Advanced Care Coordination Program, teaching medical trainees, and participating in a shared call rotation with flexible scheduling. Candidates must be board certified or eligible in Geriatric Medicine or related fields, possess Michigan licensure or willingness to obtain it, and have experience with geriatric or medically complex populations.
A well-established healthcare organization in Grand Rapids, Michigan is seeking a full-time Geriatrics Physician (1.0 FTE) to join their dedicated team caring for complex older adult populations across local nursing facilities and advanced care programs.
Position Overview:Provide care for geriatric patients across eight local nursing facilities
Support an Advanced Care Coordination Program focused on home-based and transitional care

Board Certified or Board Eligible in Geriatric Medicine
OR
Board Certified in Internal Medicine or Family Medicine with Geriatrics experience REQUIREDMust become Board Certified within 2 years, if currently eligible
Experience working with geriatric or medically complex patientsREQUIRED
Michigan medical license (or willingness to obtain) REQUIRED
DEA, BLS, and ACLS (or willingness to obtain) REQUIRED
New graduates with Geriatrics Fellowship training are welcome
