We are seeking a dedicated physician to provide primary care rounding at post-acute facilities in the Wilmington, DE area. This is a full-time role with schedule flexibility, allowing you to build a rewarding practice while maintaining work-life balance.
Position Details:
Location: Wilmington, DE
Setting: Skilled Nursing Facilities (SNF) and Assisted Living Facility (ALF)
Schedule: Full-time, 5 days per week
Rounding flexibility: Set your own schedule during daytime hours
Responsibilities:
Conduct primary care rounding in SNFs and an ALF
Manage chronic diseases and acute medical conditions
Develop and implement care plans
Collaborate with interdisciplinary teams and facility staff
Provide mentorship and oversight to nurse practitioners
Engage with patients, families, and facility leadership to ensure quality care
Participate in care coordination and network quality initiatives
Qualifications:
MD or DO with board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics
Active Delaware medical license (or ability to obtain)
Experience in post-acute care, geriatrics, or chronic disease management is a plus
Strong communication and collaboration skills
