Job Description
American Traveler is seeking a Physical Therapist for a home health hospice position, requiring at least 1 year of recent experience and proficiency with HCHB EMR.
Responsibilities
- This is a home health setting serving hospice patients
- Patient population is primarily geriatric
- Average daily caseload is 5-7 patients, to be confirmed during the interview
- Proficiency with HCHB EMR is required
- Day shift schedule: 5x8 hour shifts, 8:00 AM – 5:00 PM
- On-call rotation required every 6th weekend
- Territory may extend up to a 60-mile radius from the office
- No local candidates accepted
- Primary responsibilities include providing physical therapy treatment and documentation for home health hospice patients
- Mileage reimbursement provided at the IRS standard rate
- iPad provided for work documentation; cell phone not provided
- Dress code is scrubs of any color; jeans are not permitted
- No call-ins are permitted
- First-time travelers are not accepted for this assignment
- Candidates must be prepared to cover territory within a 60-mile radius of the office
Requirements
- Current PT license in hand is required if applicable
- Minimum of 1 year recent home health or hospice physical therapy experience
- Current BLS certification required
- Proficiency with the HCHB EMR is required
- Oasis experience required for staff completing SOC
- Ability to provide timely documentation and communication
- Must be flexible with travel territory
