Position Overview
We are seeking an experienced Primary Care Nurse Practitioner to provide on-site medical assessments for residents in a nursing home setting. This is a part-time opportunity ideal for a provider looking for consistent, predictable hours without call or weekend responsibilities.
Responsibilities
Conduct comprehensive and follow-up medical assessments for nursing home residents
Manage chronic conditions and address acute medical concerns as needed
Perform routine evaluations, medication reviews, and care plan updates
Collaborate closely with nursing staff and facility leadership
Document all patient encounters accurately and in a timely manner within the EMR
Schedule & Coverage
On-site coverage 2 days per week
No nights, weekends, or call
Flexible scheduling depending on provider availability
Qualifications
Certified Nurse Practitioner (NP) with Primary Care focus (FNP, AGNP, or equivalent)
Active state NP license (or ability to obtain)
Experience in primary care, geriatrics, or long-term care preferred
Strong assessment, documentation, and communication skills
