Requirements (boards, license):
- Board certified or board eligible within 5 years of training, must have 1+ year of experience, if boards lapsed board eligible not acceptable,
- Will consider 2 MP cases in the last 10 years (must outline state, amount and settled year)
- No license sanction or licensure issue ever, compact license is considered (must get a CSR IN license with the full license and recommend apply for both at the same time if going compact route)
