Job Title: Physical Therapist

Location: Spokane Valley, WA

Schedule / Job Type: Full-time, Days

Key Responsibilities: • Provide physical therapy services to patients in their homes, ensuring they receive individualized care and treatment plans.

• Conduct assessments, develop and implement treatment plans, and monitor patient progress.

• Collaborate with healthcare teams to ensure comprehensive care and coordination of services.

• Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care delivery.

• Maintain accurate and timely documentation of patient interactions, treatment plans, and progress.

Required Qualifications:

• Must have a valid Washington state license as a Physical Therapist.

• At least 1 year of recent skilled home health PT experience within the last 3 years.

• Current BCLS certification.

Preferred Experience / Skills:

• Experience working with adult patients with various medical conditions.

• Familiarity with OASIS documentation tools.

• Ability to work independently and as part of a team.
