The Hospitalist Nurse Practitioner will manage transfer calls from outside facilities requesting higher levels of care or specialty services and coordinate patient acceptance or declination within the Deaconess Health System. This role involves working closely with Transfer Center and Bed Placement RNs to route patients to appropriate facilities or care levels, primarily based at the Deaconess Midtown Command Center. Candidates must hold relevant state licensure and certification, with shifts including both day and night 12-hour schedules.
