Job SummaryThe Physical Therapist is responsible for evaluating physical therapy needs and delivering comprehensive, medically prescribed treatment to patients in a supervised rehabilitation setting. The role includes developing individualized treatment plans based on patient capacity and setting realistic, achievable goals.
Responsibilities- Document and record patient conditions and provide education to patients and families regarding physical therapy plans.
- Perform initial assessments and evaluations for designated patient populations.
- Review medical charts to obtain relevant clinical information and clarify referrals when needed.
- Select appropriate assessment tools and techniques to accurately determine patient needs and create optimal treatment plans.
- Request treatment orders when needed for unfamiliar diagnoses.
- Develop and implement discharge plans in collaboration with patients, caregivers, and care team members.
- Bachelor’s Degree in Physical Therapy
- Active New York State Physical Therapy License
- Strong communication and interpersonal skills
- 0–1 year of experience as a Physical Therapist
- Bilingual (Spanish) strongly preferred
Salary: $95,000–$100,000
