What You’ll Be Doing:

•  Providing personalized, one-on-one care to patients in their homes — no packed schedules or double-booked visits
•  Developing and carrying out thoughtful treatment plans designed to help patients regain independence
•  Working closely with your mentor and interdisciplinary team for guidance and collaboration
•  Learning the ropes of home-based care while building confidence and clinical judgment
•  Documenting your work in an EMR system that’s actually user-friendly
