Certs/Licensure Requirements Wisconsin Pharmacy Licensure – in good standing
AHA BLS required
What areas will they cover (ER, ICU, Inpt, Outpt, etc.) Critical access hospital with ED, ICU, Med/Surg, Behavioral Health Infusion Center, Surgical Center and infusion center. Likely will focus on clinical area, with less focus on infusion center Common Duties/Job Requirements/Skills - Review of medication orders for drug therapy and evaluate appropriateness of use, dose, dosage form, regimen, therapeutic duplication and drug interactions -Provide pharmacokinetic/anticoagulation dosing -Participate in the provision of pharmaceutical care through clinical services including medication reconciliation and discharge medication education - Pharmaceutical compounding oversight including USP regulatory/compliance guidelines (USP 797 and USP 800 focused) Experience or 'Must have' skills EPIC and Automated Dispensing Cabinets Pharmacy Manager/Supervisor Pharmacy Director – Amanda T Pharmacy Techs? Yes Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) Shifts – 0630 to 1900 (12-hour with 30” unpaid lunch) Or 0800 to 1600 Self-scheduling possibility; willing to ensure work-life balance for right candidate Weekend Requirements Yes, every 4th Holiday Requirements Yes Is there an on-call requirement?
