Job DescriptionWe are seeking a dedicated and compassionate Full-Time Physical Therapist to provide high-quality rehabilitation services in a 45-bed, family-friendly Skilled Nursing Facility. The PT will work 6–8 hours per day and is responsible for delivering comprehensive patient care, including admission evaluations, treatment sessions, progress notes, and discharge documentation. This role is ideal for a therapist committed to improving residents' mobility, function, and overall quality of life through evidence-based therapeutic interventions.
Responsibilities- Perform comprehensive admission evaluations and develop individualized treatment plans.
- Provide daily therapeutic interventions to improve strength, mobility, endurance, and functional outcomes.
- Complete all required documentation, including progress notes and discharge summaries, in accordance with facility and regulatory standards.
- Collaborate with an interdisciplinary care team to ensure cohesive and effective treatment strategies.
- Communicate regularly with patients, families, and staff to support education and care goals.
- Maintain accurate records using Point Click Care (PCC) EMR system.
- Adhere to all state, federal, and professional practice guidelines.
