About the Role:
Seeking experienced specialty physicians to provide consultative care for residents in nursing home settings. This role focuses on telemedicine consultations, with occasional in-person visits, particularly for initial assessments. Specialists in Psychiatry, Cardiology, Endocrinology, Nephrology, Neurology, and Infectious Disease are encouraged to apply.
Key Responsibilities:

• Provide telemedicine consults for nursing home residents in your specialty.
• Conduct initial in-person assessments as needed.
• Collaborate with nursing home staff, primary care providers, and other specialists to develop and implement individualized care plans.
• Review patient histories, diagnostic tests, and lab results to provide expert recommendations.
• Ensure timely documentation of consults and recommendations in the medical record.

Qualifications:

• Board-certified or board-eligible in your specialty.
• Active NY medical license or eligibility for licensure.
• Strong communication skills and ability to collaborate in a multidisciplinary team.
• Prior experience in a nursing home or long-term care setting is a plus.
