Occupational Therapist (OT) – Rehabilitation | Contract
We are seeking an experienced Occupational Therapist (OT) for a long-term contract assignment in a rehabilitation-focused Skilled Nursing Facility (SNF) setting. This role is ideal for a clinician with strong SNF rehab experience who is flexible, patient-centered, and comfortable working a variable schedule based on operational needs.
Position Details:
- Job Type: Contract
- Assignment Length: 26 weeks

- Position Urgency: Normal
- Schedule:
- Department operates 7 days per week from 7:00 AM – 4:30 PM
- 5-day work week with possible weekend coverage (Sunday–Thursday or Tuesday–Saturday)
- Hours per two-week period may be up to 88 hours
- No guarantee of hours or shift preference

Key Responsibilities:
- Perform diagnostic evaluations and occupational therapy assessments
- Develop, implement, and modify individualized treatment plans for adult patients
- Provide comprehensive occupational therapy services for habilitation and rehabilitation
- Treat a full range of cases within the scope of the program
- Document evaluations, treatments, and patient progress accurately in the EMR
- Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
- Maintain compliance with clinical, safety, and professional standards
- Be available to work shifts, weekends, and holidays as required by operations
Required Qualifications:
- Active Hawaii Occupational Therapist license (must be active by start of assignment)
- Minimum of 2 years of occupational therapy experience with adults in one or more of the following settings:
- Skilled Nursing Facility (SNF)
- Acute Care
- Rehabilitation Hospital
- Outpatient
- Primary experience in SNF rehabilitation strongly preferred
- Strong clinical judgment, time management, and communication skills
Certifications:
- Basic Life Support (BLS/BCLS)
Additional Information:
- Candidates without an active Hawaii license at submission must apply upon offer acceptance
- EMR experience preferred (system used: Meditech / Epic as applicable)
- Must be able to arrive prior to assignment start to complete required employee health onboarding
