Job Description- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans.
- Perform comprehensive patient assessments and monitor ongoing health status.
- Administer medications, treatments, wound care, and other skilled nursing services.
- Educate patients and caregivers on disease management, medications, and home care needs.
- Accurately document visits, assessments, and care provided in a timely manner.
- Communicate effectively with physicians, case managers, and interdisciplinary team members.
- Ensure compliance with agency policies, state regulations, and patient safety standards.
Requirements- Active Registered Nurse (RN) license (state-compliant).
- Previous Home Health nursing experience preferred.
- Strong assessment, clinical judgment, and independent decision-making skills.
- Reliable transportation for patient home visits.
- Excellent documentation and communication skills.
