The position is for a board-certified or board-eligible Palliative Care Physician to provide expert consultations and symptom management in both inpatient and outpatient settings. The role involves collaborating with a multidisciplinary team to develop individualized care plans, facilitating patient and family discussions on prognosis and goals of care, and participating in educational and quality improvement initiatives. Facility is seeking Palliative Care Physician to join our esteemed team. - Provide expert palliative care consultations and follow-up care to patients in various settings, including inpatient and outpatient.- Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and chaplains, to develop and implement individualized care plans.- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic evaluations.- Manage complex symptoms, including pain, nausea, and respiratory distress, to improve the quality of life for patients.- Facilitate discussions with patients and families regarding prognosis, goals of care, and advance care planning.- Educate and support healthcare providers, patients, and families about palliative care principles and practices.- Participate in quality improvement initiatives, research, and educational activities to advance the field of palliative care.- Competitive salary and comprehensive benefits package, including health, dental, and vision insurance.- Generous paid time off and CME allowances.- Retirement plan- Relocation Reimbursement- Student Loan Reimbursement Eligibility- Supportive and collaborative work environment.- Required: BC/BE in Hospice and Palliative Medicine, NH state medical license.
