Mandatory Requirements:

• Board Certified (No BE), will accept IM/FM with minimum of 2+ years of recent ED experience - Required
• MI License - Required at time of clear, no exceptions - not accepting IMLC - Required
• Certification Requirements: ACLS, BLS, PALS - Required
• Provider must be local - REQUIRED, no exceptions
• Must have clean background for quick credentialing - REQUIRED no exceptions

Employee Health Requirement:

• a. Flu vaccine for 24-25 season
• b. Covid documentation - If 24-25 booster has not been received a declination is needed
• c. TB - 2 step skin test within 12 months or Quantiferon within the last 6 months
• d. TB Questionnaire
• e. Hep B - titer with quantifiable numbers or declination
• f. MMR - vaccine series or titers with quantifiable numbers
• g. Varicella - vaccine series or titers with quantifiable numbers
• h. TDAP - adult vaccine (within the last 10 years)
